For patients under the age of 18
A parent or legal guardian must accompany new patients under the age of 18 to their initial visit. The parent or legal guardian may use a Minor’s Consent for Treatment form to designate another adult to accompany an existing patient during follow-up visits. This form can be obtained during check-in.
Care & services
You will receive results-focused treatment. Our care team is committed to delivering that experience throughout your visit. To do so, we may ask for the following:
- Additional filming. Even though you have provided X-rays to us, our physicians may require more information to properly diagnose or prepare you for surgery or other treatment.
- More perspective. To gain the most comprehensive view of your condition, your physician may consult a professional from another specialty.
- Your patience. We always do our best to meet appointment times, but our physicians may need more time than anticipated with other patients. Know that we will give the same level of personalized care to you.
- Occasional flexibility. Our physicians are specialists who provide on-call services at many local hospitals. It is possible that your physician may be called for emergency surgery during clinic hours.
We look forward to developing a personalized care plan that keeps you active.
