If you have forgotten your username or need general support, please contact our support team at (813) 979-0501, available Mon-Thurs 8 AM – 5 PM.
If you’ve forgotten your password, you can select “Forgot password?” from the main login page of the portal.
Enter the online patient code on your patient statement in the field to make a one-time payment. Patients can find their unique payment code in your FOI account statement. Having trouble locating your code? Click “Where can I find my code” on the patient portal login screen.
Log in to the patient portal using the above fields. Once you are logged in, you will be able to:
- send a message to the medical team
- request prescription refills
- check the status of your bill
