The Florida Orthopaedic Institute care team will provide you with the highest quality care before, during and after your visit. Use these resources to prepare for your upcoming visit.

Please bring the following items to your appointment to ensure that our physicians will be able to see you:
- Insurance card
- Photo ID
- Current medications list
- MRI or CT report and film/CD
- X-rays taken of your injury or condition
Once you arrive, we will:
- Make copies of your insurance card and ID
- Verify that all information in our database is accurate
- Provide you with the necessary forms for you to complete and sign
- Collect your copayment
Log in to your patient portal to request prescription refills, access labs and health information, contact your care team, or view upcoming appointments.
